Brooker Memorial Dental Center, Torrington Public Schools and Delta Dental are working together to provide a school dental program focusing on preventive dental care in each elementary school. Children in Kindergarten through fifth grade will be offered dental cleanings, caries risk assessments, and fluoride at their school, once during each school year. The program is open to all students at no cost to families, whether or not your child has dental insurance.

A registered dental hygienist from The Brooker Memorial Pediatric Dental Center will provide these services to your child. If your child needs to see a dentist for additional treatment, this will be detailed on the report our hygienist will mail to you after your child’s visit. If your child maintains regular dental appointments with your family dentist or Brooker Dental, you are still entitled to participate in this program. The goal of this program is to make sure children have access to dental care. We know it can be difficult to take time off from work for your children’s healthcare appointments.

All children in grades kindergarten through fifth will participate in an educational school dental care presentation by Brooker’s Dental Staff. This will introduce them to the dental program and get them excited to see the hygienist. Medical and permission forms are below, at your school website, or by requesting a paper copy from your school nurse. Spanish versions are also available. Please fill out ALL requested information asked on the following forms AND sign the required areas.

As always, we will be adhering to our strict dental sterilization procedures. All of our school settings are licensed by the State of Connecticut Health Department.
